Our New Patient Package:

• A Comprehensive exam to determine your current state of dental health and/or cosmetic need.
• Dental x-rays
• A regular cleaning
• Treatment Plan and explanation

If you would like to schedule a visit to our office, follow these simple steps:

1. Call

Call us to 502.241.5055 and talk to an actual human being or click here for our Request An Appointment form.

2. Print

Print out New Patient form and Medical History form and fill it out. Or, you may show up 15 minutes prior to your appointment and fill this out these forms in our comfortable lobby.

3. Show up

Show up for your appointment and receive our New Patient Package. You're now on your way to dental health and having your very best smile!
